Confidential Client Intake Form

Appointment Procedures

1.) Contact me through email or phone to arrange an appointment. Please Keep in mind my time zone is Hawaii Pacific time (GMT -10).

2.) Fill out Confidential Client Intake Form. There are two pages total, please answer as much as you are comfortable with.

3.) Please Fax the Confidential Client Intake Form back to me at least a day prior to the appointment so we can effectively work on the issue during the session.

Helpful things to do prior to the appointment to help us both to be more effective during the session.

1.) Learn more about EFT at www.EFTuniverse.com. The more you learn about EFT ahead of time the more effective your session will be.

2.) If you haven’t done so already, I’d like you to watch the highly acclaimed documentary, “The Secret” before our session. It will greatly facilitate our session and speed up your healing process. You can watch this powerful film online for $4.95 at www.TheSecret.tv or order the DVD for $29.95. You’ll understand why I feel this film is so valuable to your progression when you see it.

3.) Drink a lot of water. This will help with clearing the energy blockage a lot easier (energy naturally flows easier in a hydrated body).

4.) Make sure to have your headset or speaker phone setup prior to PHONE SESSION. This allows you to have your hands free for tapping.
